The Crossover: Transition Planning for Multi-System Engaged Youth explores the unique challenges and opportunities involved in supporting youth who are simultaneously involved in child welfare, juvenile justice, and behavioral health systems.  Through an interactive and solution-focused approach, participants will examine the barriers that often prevent effective cross-system collaboration and the impact these barriers have on youth outcomes, including housing stability, employment, education, wellness, and permanency. The session highlights strategies for reducing siloed service delivery, strengthening partnerships among professionals, and creating coordinated transition plans that center youth voice and choice. Participants will identify key stakeholders across systems, explore practical frameworks for collaborative case planning, and learn how to develop comprehensive transition plans that promote long-term success for vulnerable youth. Drawing from research, real-world practice, and years of experience supporting transition-aged youth, this presentation provides actionable tools to help practitioners, educators, social service professionals, and behavioral health providers improve outcomes for multi system engaged youth through intentional collaboration and shared accountability.
